All the information in My Patient Portal comes from your electronic health record at Hammond-Henry Hospital. This portal provides you access to the most accurate, up-to-date information available. It is important to note that you may see your result faster than your provider. However, your provider will follow up as expediently as possible.

My Patient Portal allows you to:

  • View imaging results
  • View billing statements
  • Request appointments
  • Track visit history
  • Pre-register for appointments
  • Review healthcare information from Inpatient and Emergency Department visits
  • Communicate by message with members of your Primary Care Provider's team

Enrollment Directions:

In order to self-enroll, you will need your medical record number and have a valid email address on file with the hospital. IF enrolling a child please check out our FAQ page.

If you do not know your Medical Record Number please call Health Information Management at 309.944.9108, or you may alternatively complete and mail the paper Request for Access Form. Once the form has been received and processed, you should receive a phone call detailing the next steps in order to verify your identity. At that time you will be able to choose your username and password. Processing normally takes 72 business hours after our office receives the completed form.
