Protection of patient information is more important than ever before. You can be confident that we are doing everything we can at Hammond-Henry Hospital to keep your records safe. Your medical information is as important to us as our own.

How do I get my medical records?

There are a couple of ways to attain your medical records. For a condensed version, you can enroll for online access to My Patient Portal. Should you need a more in-depth copy of your medical records, you may contact our Health Information Management Department at 309.944.9108. One of our staff will be able to help attain your information stored at Hammond-Henry Hospital. Utilizing electronic medical records, we are able to quickly gather your records. You will need to complete an authorization for release of protected health information form and show a photo ID. We will then have the records available for pick-up or mail them to your home. Our offices are open Monday thru Friday 7 a.m. - 4:30 p.m.

How much does it cost to get my records?

The cost of attaining records depends on what you need. Costs are based on Illinois law and decided case by case.

Can you email my records to me?

Though email is very convenient, it can be unsafe to send important documents over email. To ensure patient identities and information stay safe we have made it our policy to not email records at this time.

Where do I get my radiology results?

No matter the nature of your records, you are always welcome to contact our Health Information Management Department, but if you wish you may contact the Imaging Department directly at 309.944-9135 for any imaging records.

Do you keep advance directives and medical POA information?

Any information regarding advance directives, healthcare power of attorney, living will and the like are stored within Health Information Management. If you have any questions about setting up any of these types of documents, please contact our Social Services Department at 309.944-9170.
