Patient Message Form

Thank you for sending an e-mail message to one of our patients/residents currently at Hammond-Henry Hospital. Your message will be hand delivered by one of our caring volunteers. The patient must currently be an inpatient of the hospital in order for the e-mail to be delivered. We are not able to send replies back to the sender from a patient. E-mails are only delivered Monday-Friday. For your security, please do not include any sensitive or private information. If for some reason the patient you are trying to reach has already been discharged, Hammond-Henry Hospital does not assume any responsibility for getting the message to that patient.
