72 hours a pay period – shares 24-hour weekend on call rotation

Job Description

 Nurse will:

  • Provide direct patient care in home setting taking role as case manager
  • Work in multi-disciplinary team to provide full range of home health services including assessing, planning, coordinating and evaluating plan of care under the orders of a physician
  • Participate in team-based quality improvement, supervises C.N.A.

Qualifications

  • Current Nursing license from the State of IL
  • Current BLS
  • Valid Illinois driver’s license with dependable transportation
  • Proof of auto insurance
  • Ability to work independently, make sound decisions and proficient in computer skills.
  • Proficient in IV’s insertion, lab draws and works effectively in a team atmosphere
  • Excellent communication skills and portray a positive attitude.
  • Able to lift up to 70 pounds occasionally and have the ability for frequent stooping, kneeling, bending, twisting and turning
  • Position requires continuous walking and standing
  • Minimum of 2 years’ experience in an acute care setting with home health experience preferred
