* Please upload a photo or video of yourself doing your job (just make sure it's light enough and not blurry.) If a patient is with you, download the Photograph/Video/Audio Record Release & Consent Form from the Intranet under Forms/General Forms. When completed, send it to marketing. We can only use the patient photo with this release.

* By submitting this information, you give Hammond-Henry Hospital permission to use the information and photo/video for promotional purposes. Your photo and comments will be subject to use in any marketing by HHH. There is a possibility that you will not see the promotional material before it is used. Do you agree to this?
