Description:  64 hours a pay period. Assists in directing and supervising all aspects of SNF/ICT services, functions and activities in accordance with established policies, procedures, applicable laws and regulatory requirements. Responsible for all MDS scheduling, organizing, interdisciplinary team work in completion of MDS and care plans; gathers MDS information as needed, tracks completion, billing and transmission of MDS. Completes charts audits, Medicare certifications and re-certification requirements; assists with diagnosis/problem lists and chart thinning. Takes on call.
