In order to self enroll you will need your medical record number and have a valid email address on file with the hospital.
If you do not know your Medical Record Number then you must fill out an online Request for Access Form, or you may alternatively complete and mail the paper Request for Access Form. Once the form has been received and processed, you should receive a phone call detailing next steps in order to verify your identity. At that time you will be able to choose your username and password. Processing normally takes 72 business hours after our office receives the completed form.
